LONDON, UK, Aug. 24, 2026 (GLOBE NEWSWIRE) — Healthcare professionals from across the United Kingdom and around the world will have the opportunity to earn recognised Continuing Professional Development (CPD) credit while exploring the future of integrated healthcare at the inaugural International Case Management Conference (ICMC), taking place in London on 17–18 September 2026.

Designed to advance professional education and foster international collaboration, ICMC will bring together physicians, nurses, case managers, social workers, allied health professionals, healthcare leaders, policymakers and researchers to examine the evolving role of case management in improving patient outcomes, strengthening care coordination and transforming healthcare delivery.

“Having seen firsthand how coordinated, patient-centred case management can improve outcomes, patient experiences and healthcare efficiency, I know the value of learning from others doing this work. So, I encourage healthcare professionals and leaders to attend the upcoming International Case Management Conference in London to exchange ideas, build connections, and learn from colleagues bringing real-world experience from across the UK and internationally,” said Maria Virkki, MD, PhD, EMBA, Strategic External Development Lead at Pfizer in Helskini, Finland, and member of the ICMC Organising Committee.

The conference also aligns with the priorities outlined in the NHS 10-Year Health Plan, including shifting care from hospitals into communities, advancing integrated and digitally enabled care, strengthening prevention, and improving outcomes through greater collaboration across the health and care system. These themes are woven throughout the conference programme, providing attendees with practical strategies to help prepare for the future of healthcare.

The conference has been approved by the Federation of the Royal Colleges of Physicians of the United Kingdom for 10 Category 1 (External) CPD Credits. The educational programme has been developed to support the continuing professional development of healthcare professionals across multiple disciplines. Participants are encouraged to claim CPD credit or hours in accordance with the requirements of their respective professional bodies and employers. All attendees will receive a Certificate of Attendance documenting their participation.

The two-day programme will feature internationally recognised experts presenting evidence-informed strategies, innovative models of care and best practices that address today’s most pressing healthcare challenges. Attendees will have the opportunity to exchange ideas, build professional networks and gain practical knowledge to support improvements in patient care across diverse healthcare settings.

Effective discharge planning is fundamental to delivering safe, coordinated care. As health systems seek to reduce avoidable readmissions, improve patient flow, and create more seamless transitions between acute, community, and primary care, multidisciplinary collaboration has become increasingly essential to achieving better patient outcomes.

Among the newest additions to the programme is “Improving the Discharge Process: Collaborative, Patient-Centred Approaches to Care Transitions,” presented by Dr Corné Hurter, MBChB, MSc, Medical Director of Services & Delivery at AXA Health.

In addition to expert-led educational sessions, attendees will have opportunities to engage in international networking, exchange best practices, and explore innovative approaches to healthcare delivery from organisations across the United Kingdom, Europe, and the United States.

ICMC represents a landmark opportunity for healthcare professionals to gain practical knowledge, build global connections, and contribute to the future of integrated, patient-centred care.

Healthcare professionals interested in attending are encouraged to register early before pricing increases on 28 August. Learn more and register today at smartcareconference.com.
